We are listed as in-network with the above insurance plans. Please call your carrier to discuss cost of care that include payments you may be responsible such as deductibles, co-pays, and/or co-insurances. We are out-of-network with the above insurance plans. Swarthmore Student Health is United, but has a low deductible and a small co-insurance so students may be seen at an affordable rate. Please call your carrier to discuss cost of care as some plans do have percentages of the care covered at reduced rates. Out-of-network benefits will likely have differing rates than in-network such as larger deductibles, co-payments, and co-insurance percentages. Keystone plans are HMO managed care plans requiring you to see a specifically designated provider to receive benefits. Ask about your physical therapy "capitation site" in order to confirm where your benefits would be accepted. Your PCP will choose what PT site you need to receive care.
We are not in-network with Medicaid plans. Medicaid enrollees must be seen by in-network providers and out-of-network is unfortunatley not possible for these plans.
